I've had this unusual 8" diskette since the late '70s when I worked at Noakes Data Communications in Irving Texas.
It's been protected in my stash during that time and is in pretty good shape.
As you can see the index hole is on the outside edge and there appears to be approximately 8 sector holes.

I've been told it's a Minnow disk.
